I'd say that your problem is your transmission. I'm speaking from past experience. I own a '94 F150 w/5.8 & AT with 136,000 miles. My tranny started having the same problem of not going into OD at around 100,000 miles. At 109,000 the tranny finally quit. I have since installed a Jasper HD AOD w/HD Cooler and it actually runs better than originally, plus it helped to increase my fuel mileage. Before I purchased the Jasper, I spoke with the Jasper techs and this is what they told me: "The Ford E4OD was produced to run more efficiently in the F150's. The problem is, they're not designed to handle any towing torque whatsoever, nor are they setup to handle high mileage. We do 28 different upgrades to this transmission for durability, maximum towing power and more fuel efficiency. Ford has since come to us and contracted with us to build all of their E4OD trannys from now on. We offer a 75,000 mile warranty on this transmission that's good at any shop in the United States."
Bear in mind that these aren't cheap, but if you're going to keep the truck, I would highly recommend going this route. I had mine installed out the door for $3,100 and that includes a HD Cooler that comes with the tranny, a NEW Torque Converter that I purchased, fluids and labor. I haven't regretted the expense at all, and it pulls my 22 Ft. travel trailer better now than it ever has. I hope this helps and good luck.
